- What to expect during your first visit
During your first visit, we will :
- Review your dental and medical history forms
- Take x-rays and digital photographs of your teeth and mouth
- Complete a comprehensive examination
After your examination is complete, the doctor will meet with you to create your own individual treatment plan, and answer any questions that you may have.
Regular checkups are an important part of maintaining a health smile.
The American Dental Association recommends visiting your dentist every six months for regular checkups and routine teeth cleanings to keep your teeth and gums healthy,
We understand that you have a busy schedule, so when you are preparing to visit the dentist, please remember that your appointments can last between 45 to 90 minutes.
- What to bring to your first appointment
To provide you with prompt dental treatment, please be sure to bring the following items when you visit our practice for your initial visit:
- Insurance information/Card
- Photo identification
- Any medical records pertaining to your visit
- Legal guardian authorization form (if a minor has to come with legal guardian)
Please arrive 15 minutes ahead of your scheduled appointment time to fill out necessary paperwork. If you would like to complete your paperwork beforehand, you may print and fill out the appropriate forms found below.
- What to do to reschedule your appointment
When you schedule an appointment, we reserve that time especially for you. We understand that situation arise and you may need to reschedule. As a courtesy, we ask that you contact our office 48 hours in advance to change an appointment. You may be charged a fee for missed or broken appointment.
